Expectations and Responsibilities Prior To Treatment
1. Documentation
Shortly after signing up with AIM you will receive an email with documents you will need to read, answer, and ultimately execute. These documents include:

a} Instructions on procedure,
, b} a general informed consent,
c} a request for medical records/release,
d} a general health and allergies questionnaire., and
e} Accepted forms of payment for treatment

You will be required to mail us the hard copy of thes executed documents. Once AIM is in receipt of the executed documents a representative of AIM will contact you using your preferred means of communication to collect the clinical
fee. ​
Please note: In the even you do not have access to a printer, please use the contact form to request a hard copy of the forms to be sent to your address.

2. Compiling Master Patient File And Initial Screening
Once AIM is in receipt of the necessary documentation and clinical fee, we will compile a master medical file for the patient to provide to the treating physician who will conduct a pre-treatment screening.
During the screening process you may be contacted to provide more information to assure you are a proper candidate for the treatment.
Once the treating physician concludes the review your medical history and current state of health, You may receive a brief call or email from AIM to request more information.
Once AIM completes the screening you will either be accepted or denied treatment at the sole discretion of the treating physician. AIM reserves the right to deny any applicant access to the treatment. Denials will be based on current understanding of side effects and expectations of benefits of treatment.
If ACCEPTED, you will be asked to schedule a consultation to begin treatment, or
If DENIED, you will be refunded the fee, minus a $100 administration fee for the screening..
The Consultation and Treatment Plan
The patient will be directed to schedule an appointment for a video consultation with an attending physician. The primary purpose of the consultation is to narrow down a realistic treatmen and monitoring plan based on the patients personal objectives, overall health, medical history, and finances (which can have more of an effect on how close AIM will be able to monitor the patient's treatment and prognosis). The patient will be informed on the two treatments advocated by AIM, treatments believed industry wide to be tthe "gold standard" in treating aging as a disease..

AIM's consultation goal will be to assure each patient fully understands mechanism underlying the treatments and why AIM advocates for the same.
